Dear Valued Patient:
I hope this letter finds you in good health. I am writing to inform you that my last day with Colorado Springs Urological Associates, LLC (“CSUA”) is Wednesday, June 14, 2023. My family and I are moving to Michigan and I will be joining a CSUA affiliated practice based in Michigan.
Although I am moving out of state, my colleagues at CSUA will continue seeing patients at our two Colorado Springs locations located at 6071 East Woodmen Road, Suite 300, Colorado Springs, Colorado 80923 and 1644 Medical Center Point, Suite 200, Colorado Springs, CO 80907, and are available to assume your urological care. If you would like to make an appointment with one of my colleagues, please call our office at (719) 531-7007. You have a choice in health care providers, and we hope you continue to choose CSUA. If you choose not to transition your care to one of my CSUA colleagues and would like to find a new provider, you can request a copy of your medical record by contacting our office by mail at 6071 East Woodmen Road, Suite 300, Colorado Springs, Colorado 80923 or by phone at (719) 531-7007. Regardless, your medical records will continue to be stored at our medical offices in accordance with Colorado and Federal law.
